Medical Records
A patient, parents of minor patients, or caregivers acting on the behalf of a patient (i.e.Healthcare Power of Attorney), may inspect and/or obtain a copy of their medical records, or have copies of medical records sent to another provider or facility.
Obtain Printed Copies of Your Medical Records
The Release of Information Office is located in Building B next to the Medical Office Building Parking Garage. Patients may come to this location to request copies of their medical records. If you have any questions or to confirm the location before you arrive please call 706.320.2752.
- Photo Identification is required.
- Complete and sign the release of information authorization form.
All medical records are maintained at St. Francis hospital, located at 2122 Manchester Expressway, Columbus GA 31904.
If you need to request your medical records please call 706.320.2752, select Option 1, and you will speak to the Release of Information Department. They will be able to provide you with instructions on how to obtain an “Authorization for Release of Information” form or you can download and print it below. If you need to stop by the office the hours of operation are Monday – Friday, 8:30 a.m. – 5:00 p.m.
